One month prior to your intended leave of absence

First obtain approval from the chair of your department or your academic supervisor. Then submit this form along with relevant documents proving your personal circumstances.

If you choose "Study abroad" as the reason, the following documents should be attached.
Study abroad proposal form
・A copy of acceptance letter from the host institution
For students who choose "Study abroad" as the reason and wish to have the credits earned at a foreign university recognized as the credits at Tokyo Tech, the following documents should be attached in addition to the documents listed above.

First obtain approval from the chair of your department or your academic supervisor and then submit this form. In case of a leave of absence due to medical reasons, attach a medical certificate, preferably completed by an in-house medical doctor at Tokyo Tech.
