Points to Note
Application Method
1. Applying in Person <Currently NOT available>
・ As a general rule, you, the alumni must apply in person. Prepare to show your identification (such as a driver's license, health insurance card, or passport) when you apply. Applications can be lodged at either the Ookayama or Suzukakedai Campus.
・ When you apply through a proxy on your behalf, the proxy must provide a Proxy Authentication letter and their own identification.
・ If you wish to receive the certificate by post, provide a "stamped self-addressed envelope" for sending the certificate. (The envelope shall show the name and address of the recipient, and have a postal stamp in the amount mentioned below.)
・ If you wish to receive the certificate in person, bring your identification with you again (and the Proxy Authentication letter when applying by proxy).
Applications can also be lodged by faxing the application form (only by the graduate themselves), but if you wish to receive the certificate by post, make sure to include a copy of your identification when sending your stamped self-addressed envelope.

2. Applying by Post
Send the followings to one of the campus offices in charge (1. Ookayama or 2. Suzukakedai.)
A. Application form Certificate Application Form (for Graduates and Students That Have Completed Their Studies)(75KB)
B. A copy of your identification (such as a driver's license, health insurance card, or passport). If you apply through a proxy on your behalf, the proxy must provide a Proxy Authentication letter and their own identification.
C.Return envelope
A self-addressed Nagagata 3 (23.5 cm x 12 cm) standard-sized envelope with a stamp pasted. When applying for multiple copies of the certificate, please use a large (non-standard size) envelope.
When you apply from overseas, please arrange the C.O.D. (Collect on delivery) payment services of logistics carriers such as DHL or FedEx by yourself.
Postal fee estimate in Japan:
Up to two certificates: 94 yen regular postal stamp (for standard size envelope)
Up to four certificates: 140 yen regular postal stamp (for standard size envelope)
For four and over four certificates: Amount of stamps that corresponds to the size (for non-standard size envelopes).
If you are in a hurry, please also attach an additional express stamp (260 yen).
Time Required to Issue Certificates
Certificates cannot be issued on the day of application.
The number of days required for issuance is indicated below. Times may be longer depending on the seasons.
*Since from March to May is a busy period, issuance of certificates may take longer than usual. We request you to apply as early as possible.
